You need a notarized consent form signed by both parents and also need a letter that should include addresses and telephone numbers of where the parents can be reached (in case of emergency) Assuming you are a U.S. citizen, since you’re planning a ROAD TRIP across the border to Canada from the U.S. you do not need a passport BEFORE June 1st 2009.You need a birth certificate and either a drivers license or a photo ID from your school.If you cross the border on or AFTER June 1st, you will need a valid passport no matter how you enter the country, by car or boat.You need a valid passport if you’re flying
